Business Associate Agreement

This Business Associate Agreement ("BAA") is entered into by and between Remedy Insights LLC ("Business Associate") and the customer ("Covered Entity"). This BAA is required under HIPAA and governs the use and disclosure of revealing health information in connection with the Remedy Insights services.

1. Permitted Uses and Disclosures. Business Associate may use and disclose Protected Health Information ("PHI") only as necessary to provide the services and as permitted by this BAA.

2. Safeguards. Business Associate will implement administrative, physical, and technical safeguards to protect the confidentiality, integrity, and availability of electronic PHI.

3. Subcontractors. Business Associate will ensure that any subcontractors agree to the same restrictions and conditions that apply to Business Associate with respect to PHI.

4. Reporting. Business Associate will report to Covered Entity any unauthorized use or disclosure of PHI and any security incident involving PHI.

5. Access and Amendment. Business Associate will make PHI available for access, amendment, and accounting of disclosures as required by HIPAA.

6. Termination. Upon termination of services, Business Associate will return or destroy PHI unless infeasible, in which case protections will continue.
